written, we will write them together. If they are not clear or not effective, we will clarify them or start from scratch and make sure they are helpful and meaningful. When we are satisfied that we have the right procedures in place, we will all work to comply with them. One last tip to help you stay on the path by avoiding the defensiveness and hurt feelings that often come with shining a light on a mistake. Instead of pointing out what someone did wrong, ask them instead, “If you could go back in time to a moment before the mistake was made, what moment would that be and what do you think you could have done differently?” In other words, try to keep the process creative rather than destructive. This process is not easy. Perhaps your company is al- ready adept at working through an effective Root Cause Analysis. If so, I would encourage you to share your suc- cess with others at association meetings and leadership roundtables.
