Our Model of Care Our team approach ensures you get exactly the therapies you need, when you need them. Our goal is to increase your activity level each day, restoring your function and independence.

SPEECH-LANGUAGE PATHOLOGISTS Our speech language pathologists (SLPs) are licensed, certified and among the most experienced in the field of aphasia treatment. SLPs will assess your thinking processes and ability to communicate, and then develop a treatment plan that focuses on understanding and remembering information, talking/ communicating and swallowing. Although each patient’s treatment plan is unique, outpatient rehabilitation typically includes a minimum of 1-2 hours of therapy, 1-2 times a week, with doctors and therapists in the specialty program aligning most closely with the source of your injury or condition. Frequency, intensity and duration of daily activity are determined by your care team and change as you progress. We encourage family participation to maximize long-term outcomes.

PHYSICAL THERAPISTS Our physical therapists (PTs) are licensed, certified and many have advanced degrees, with specialized concentrations in neurology, geriatric, orthopedic, pediatric and women’s health. Because of their specialties, therapists are able to provide expert care and treatment that is specific to your condition and needs. PTs will assess your abilities and develop an individualized treatment plan focused on a variety of activities to help with balance, mobility, strength, endurance and range of motion. OCCUPATIONAL THERAPISTS Our occupational therapists (OTs) are licensed, certified and many have advanced degrees. They work alongside physicians, and physical therapists to ensure that they are providing the best treatment for you. OTs will assess your abilities and develop a treatment plan tailored to your daily activities and routine, as well as components of these activities such as arm strength and coordination, thinking skills and safety awareness.

You’ll work one on one with experienced professionals, many with additional certification in specialties such as pain management, orthopedics, sports medicine, arthritis care, pediatrics, geriatrics and neurology. BOARD-CERTIFIED PHYSIATRISTS Physiatrists have advanced training and expertise in physical medicine and rehabilitation (PM&R). They treat patients with physical and neurological impairments due to injury, illness or medical condition. Our doctors specialize in non-surgical treatment options, giving you the best chance to have the best outcome. OUR OUTPATIENT MODEL Our experts work closely with you and collaborate with your referring physician to determine

the best treatment plan for your rehabilitation needs. Depending on your diagnosis, you may only be treated by our expert therapists.
