YOUR GUIDE TO A FUNERAL
Procedure to follow when a Medical Cause of Death Certificate (MCCD) has been issued:
Death occurs
Death at home
Death in hospital
Contact the doctor
Hospital contacts next of kin
Will doctor issue MCCD? If required ask for cremation papers to be prepared
YES
NO
Contact funeral director to arrange removal of deceased
Go to hospital: • to collect belongings of deceased • inform them if cremation is required
Collect MCCD from doctor
Register death
Does Registrar report death to Coroner
YES
NO
Registrar will: • register death • issue Green Form (registration certificate for B or C) electronically • issue Certified Copies of Death Certificate
Go to the box marked “Coroner informed” on page 11
Please note: Funeral arrangements can, if so wished, be made before registration takes place
Make contact with funeral director to arrange funeral.
