HOLES-IN-ONE Hole-in-one scorecards must be dated and signed by at least one playing witness. Include phone number and indicate if this is his/her first hole-in-one. We can only list the hole in ones if you turn in your completed form. The lucky golfer may purchase a hole-in-one bag tag.
