If you have any questions about this policy or are not certain what to do when such a contact is made, please contact the head of Marketing. 6.6 Personal Cell Phone/Mobile Device Use While CREA, LLC permits employees to bring personal cell phones and other mobile devices (i.e. smartphones, tablets, laptops) into the workplace, you must not allow the use of such devices to interfere with your job duties or impact workplace safety and health. For more information, please see the IT Mobile Device Policy provided during new hire orientation, your annual policy acknowledgments, and accessible via the Company Policy Center in Paylocity's Self Service Portal. 6.7 Mail Use You are required to limit usage of the CREA, LLC mail service to business purposes only. You may not use the Company address to receive personal mail. Do not use the Company postage meter for your personal mail. Report any suspicious packages or envelopes to building security immediately. 6.8 Personal Data Changes It is your obligation to provide CREA, LLC with your current contact information, including your name, current home address, and home telephone number. Inform the Company of any changes to your marital or tax withholding status, your status affecting your legal right to work in the United States, licenses (to the extent they are required to perform your job responsibilities), emergency contact information, number of dependents, voluntary payroll deductions, military status, or beneficiary designee. Failure to do so may result in loss of benefits or delayed receipt of W-2 and other mailings. To make changes to this information, contact Human Resources. You may also use Paylocity's Self Service Portal to electronically update some of this information. 6.9 Driving Record All employees required to operate a motor vehicle as part of their employment duties at CREA, LLC must maintain a valid driver's license and acceptable driving record. The Company may run a motor vehicle department check to determine your driving record. It is your responsibility to provide a copy of your current driver's license for your personnel file. Any changes in your driving record, including but not limited to driving infractions, must be reported to the Company. State law requires all motorists to carry auto liability insurance. It is against the law to drive without insurance. If you use your own vehicle as a part of your employment duties, you must provide management with a current proof of insurance statement or card. New proof of insurance is required every time your policy expires and renews. 6.10 Telecommuting & Work Strategy Telecommuting is defined as regularly working a full or partial workday from home or some other alternate worksite. In August 2022, CREA updated its work strategy guidelines to incorporate expectations for in-office, remote, and new employees. CREA and each department within CREA will review these guidelines periodically and may adjust to serve corporate and departmental business needs, goals, and the desires and needs of our employees. You may reference the complete policy via the Company Policy Center in Paylocity's Self Service Portal. In this work model, you will be subject to the same performance standards; work areas may be evaluated to ensure that appropriate safety standards are met. Telecommuting or a hybrid work model may be a reasonable accommodation; consult Human Resources if you are requesting telecommuting as a reasonable accommodation.
