PROGRAM DESCRIPTION:
MyGo2Support expands Crossroads4Hope’s care experience through an integrated, multidisciplinary engagement plan that uniquely addresses both the clinical and psychosocial needs of its members. By building on their previously in-person-only services, the program allows Crossroads4Hope to digitally extend whole-person care directly into individuals’ lived environments, supporting them through all stages of their cancer journey. It broadens access, strengthens the continuity of care, and reinforces the vital role of emotional and social support as essential components of comprehensive cancer care.
PROGRAM AUDIENCE: Adults with a cancer diagnosis, parents of diagnosed children, caregivers, survivors, previvors, and bereaved individuals.

Program Components:

Care Communications: Upon enrollment, participants receive text messages that communicate, promote, and support participants throughout the cancer journey. Some messages contain links to online pages with additional educational information in the form of interactive surveys, videos and music tracks (see Care Companion). Topics include caregiver tips, information for the newly diagnosed, advanced care planning, care and career, exercise/wellness tips, survivor’s guilt and treatment decisions. Care Companion: This cloud-based learning management system (LMS) provides vetted educational content that is personalized by participants’ self-expressed priorities and interests, structured into easy- to-follow sections to promote independent education, learning and self-care management.

GoMo (Secure) Chat: Much like a typical text conversation,
GoMo Chat allows live messaging between participants and their care team, enabling in- the-moment, on demand conversations.
Secure Data Collection and Reporting: Engagement and experience data is collected, deidentified, analyzed, and reported to Crossroads4Hope for ongoing experience and service improvement.
