A Holistic Approach to Managing Diabetes Diabetes is a serious chronic disease that can lead to heart disease, stroke, amputation and death. One of the most common causes of diabetes is obesity, and as of 2023, 27% of adults in Washoe County were characterized as obese. At HOPES, managing diabetes effectively means providing holistic care that includes nutrition resources and patient education. Our team of dedicated Community Health Workers help patients navigate their diabetes care journey, engaging with them to understand their unique needs and working together on a path toward wellness.

Education is Healthcare We recognize that when it comes to managing diabetes effectively, patients are more successful when they are informed and empowered to make healthy choices. Providing education through our diabetes education classes and one-on-one nutritional counseling sessions helps ensure a sustainable path to diabetes management. By offering tailored guidance, emphasizing lifestyle changes, and providing diverse tools for effective sugar level management, we’re empowering individuals to make informed choices about their health while building a supportive community with others facing similar challenges. A Prescription for Healthier Food Options Food insecurity can have a serious impact on physical and psychological health. For patients experiencing food insecurity, we provide a food prescription, which allows patients to access healthier food options from various community food pantries on a weekly basis. By removing barriers to a nutritious and balanced diet, we’re able to improve the overall health of our community.
