CRITICAL ILLNESS BENEFIT
Critical Illness Insurance pays a benefit to the participant following the diagnosis of a serious illness or event, such as Cancer, a heart attack or stroke. The plan benefit is paid directly to the insured to help offset the larger out-of-pocket expenses that accompany a serious illness, such as deductibles, copays, and lost income. The benefit does not offset other insurance.
Additional benefits are available for multiple and/or recurring conditions. There is also no pre-existing condition exclusion on this benefit. Any diagnosis of an illness must occur on or after the coverage effective date
An annual wellness benefit is also available of $50 for employees and family members after a covered health screening test. Over 40 different screenings qualify, including cholesterol screening, mammograms, and immunizations.
Employees can choose from the benefit amounts of $5,000, $10,000, and, $20,000. Spouse coverage is available at 50% of the employee’s benefit level. Child coverage is available at 25% of the employee’s benefit level. Please note, this benefit will not be available for domestic partners. Rates for this benefit will be based on enrollee age.
