If you are using modalities with risks not addressed in this Informed Consent, you will want to customize your form to advise patients of those additional risks. For an informed consent to be legal it must detail potential benefits and risks of the procedure, as well as common alternatives to the procedure-including refusing care and the risks associated with that decision. 8. Adapt the informed consent to add or eliminate procedures consistent with your own personal practice.
