Valley Health Partners Community Health Center (VHP) is a 501(c)(3) non-profit Community Health Center dedicated to providing accessible and high-quality healthcare to all, regardless of their ability to pay. We offer primary care, pediatrics, mental health, women’s health, dental services, vision care, chiropractic medicine, and specialized programs like VHP Street Medicine, which provides primary and urgent care to persons experiencing homelessness and VHP Veteran Health, which supports the health and well-being of those who’ve served. Our holistic approach includes supportive services such as transportation assistance, interpretation, health education, and care coordination, ensuring equitable and accessible healthcare for all. With a focus on whole-person wellness, VHP goes beyond, VHP goes beyond traditional care to address barriers to health and improve outcomes. ABOUT COMMUNITY HEALTH CENTERS For 60 years, Community Health Centers have delivered comprehensive, high-quality care to communities nationwide. Rooted in the neighborhoods they serve, health centers go beyond medical care—expanding access, promoting health equity, lowering health care costs, and making a lasting impact on the well-being of their communities. Health centers play a vital role in expanding access to essential primary care by removing common barriers like cost, lack of insurance, transportation challenges, and language differences. In doing so, they deliver meaningful benefits to both patients and the broader health care system.
OUR HISTORY The clinical roots of VHP can be traced back to several decades ago, when it was part of Lehigh Valley Health Network (LVHN), now part of Jefferson Health. In 2020, VHP transitioned into an independent, separate non-profit entity, focusing solely on primary and preventative care. In 2021, VHP was successfully designated as a Federally Qualified Health Center Look-Alike (FQHC-LA), commonly known as a Community Health Center. While Federally Qualified Health Centers (FQHC) receive federal funding, FQHC-LA’s, like Valley Health Partners, do not.

PROGRAM SPOTLIGHT VHP STREET MEDICINE
Founded on the idea that everybody matters, VHP Street Medicine delivers primary and urgent health care to those experiencing homelessness in the Lehigh Valley. The team utilizes a holistic patient-led approach to not only provide high-quality health care but also address barriers to access by going to the people. The aim is to offer care in terms acceptable to patients in the location of their choice whether that be in a homeless shelter, soup kitchen, on the streets, under bridges, or in the woods. Patients are offered numerous services and links to community partners – tele-psychiatry, mail services, care coordination, survival support, connection to housing services and grant-funded medical respite. VHP’s Street Medicine Respite program addresses the critical need for medical respite care for people experiencing homelessness, who face unique and compound challenges when recovering from illness or injury. Without a safe and stable environment to heal, these individuals often experience extended hospital stays, increased risk of readmission, and are frequently discharged back into unsafe or unstable living conditions, which hinders their recovery and perpetuates the cycle of homelessness and poor health. VHP Street Medicine cared for 7 medical respite patients for reasons such as cardiac catheterization, aortic valve replacement, wound care, and access to breast cancer treatment. Medical respite care not only saves cost to the health care system but also provides a bridge between hospital care and permanent housing by offering not only a safe place to recuperate but also access to medical and supportive services. By meeting these foundational needs, individuals are put in a better mental and physical space to address critical health issues such as surgery or cancer treatment, and to begin planning for their future.

PROGRAM SPOTLIGHT VHP VETERAN HEALTH
VHP Veteran Health Program supports veterans and their families throughout the Lehigh Valley and Poconos by coordinating health care and benefits services tailored to their unique needs. Staffed by clinically certified, trained professionals, the program guides clients through complex navigation and creates personalized care plans via case managers. Additionally, it connects them with community partners for support services such as compensation and claims assistance, peer support, housing, employment, financial aid, legal help, nutrition, clothing, and wellness. In June, the program hosted its first Women Veterans’ Wellness Retreat— a safe, supportive space created for women veterans. The full-day event included mindfulness exercises, yoga, massages, art making, music therapy, and more. Within this private gathering, veterans, allies, and community partners came together to express themselves freely and form meaningful connections. Together, they created a sisterhood rooted in mutual support, trust, and resilience.

MEETING THE MOMENT VHP EXPANDS ACROSS THE REGION In just four years, Valley Health Partners has made bold strides as a Community Health Center—expanding services, growing capacity, and deepening its impact. The demand for services in the community continues to grow, and expansion into Monroe and Northampton is a critical step to meet that demand more effectively. Monroe County In December 2024, VHP proudly became Monroe County’s first Community Health Center, expanding access to high-quality care in the Poconos. We welcomed five established LVHN pediatric practices—including a dental center—into the VHP network. Patients continue to see the same trusted providers in the same locations, now with the added benefits of our sliding fee discount program, medical interpreter services, and expanded social support resources. Behavioral Health VHP also expanded behavioral health services with psychiatric services at VHP Center of Hope, previously the LVHN Mental Health Clinic at 17th Street. The site offers adult psychiatric care with a focus on accessible, compassionate support. Northampton County In May 2025, we opened VHP at South Side—our first location in Northampton County. This brand-new site delivers essential healthcare to an underserved area of our community, with services including primary care, pediatrics, dental and vision care, chiropractic medicine, behavioral health and a full-service retail pharmacy This location is home to our second VHP Centro de Salud, providing culturally focused care for the Hispanic community, including treatment for diabetes, asthma, and heart disease. It also expands VHP Community Smiles to include adult dentistry—previously, only pediatric dental patients were served in Lehigh and Monroe counties.

PROVIDING OUTSTANDING CARE IN OUR COMMUNITY AWARDS & RECOGNITION
Health Center Quality Leader Awarded to health centers with overall clinical quality performance in the top 10 percent nationally. We achieved this for the second consecutive year and are the only FQHC Look-Alike in Pennsylvania to earn Gold status. National Quality Leader – Cancer Screening Recognizes health centers that meet or exceed national benchmarks for breast, cervical or colorectal cancer screening. We’re one of only two health centers in Pennsylvania to earn this award, with top-10 performance across all three screening types. National Quality Leader – Behavioral Health Awarded for meeting or exceeding targets in depression screening, follow-up care and substance-abuse intervention (SBIRT). Only six health centers in Pennsylvania received this recognition. Preventive Health Given to centers that excel in at least five of 10 preventive health measures including childhood screenings, cancer prevention and wellness counseling. We demonstrated strong performance across pediatric, adolescent and adult preventive care measures. Improving Health Care Access Recognizes significant improvements in patient access through growth in patient volume, services or clinical quality measures. We earned this by achieving a 19 percent increase in total patients while maintaining high clinical standards.

COLLEAGUE HIGHLIGHT FROM BLURRY DAYS TO BRIGHT TOMORROWS: A VISION SUCCESS STORY
Michelle Mieses, MPH SENIOR PROGRAM MANAGER, SCHOOL BASED HEALTH
During a routine parent-teacher conference day at Sheridan Elementary School, Michelle Mieses encountered a situation that would showcase the power of proactive community health advocacy . School staff approached her about a student experiencing severe vision problems—despite wearing reading glasses, the child was struggling to see clearly, frequently tripping, facing bullying from peers, and exhibiting behavioral challenges in the classroom. When Michelle learned that the child’s mother initially showed resistance to intervention, she demonstrated exceptional interpersonal skills and cultural sensitivity. Michelle strategically reframed the vision screening as a general service available to all conference attendees, successfully de-escalating tension while ensuring the child’s needs remained the priority. Working collaboratively with VHP colleagues, Elides Ruiz and Amanda Valerio, the team coordinated an emergency appointment despite fully booked schedules. When the family attempted to reschedule, the team’s dedication ensured the child was still seen that same day, with Elides personally accommodating their late arrival. The comprehensive care provided resulted in an appropriate prescription, new eyeglasses selection, and referral to an ophthalmologist for specialized evaluation. Elides further supported the family by coordinating transportation to follow-up appointments. The impact was transformational—the child’s teacher reported significant improvement in academic performance and classroom behavior once proper vision correction was in place.

When a patient experiencing homelessness needed urgent hernia repair surgery, Eric Rivers demonstrated the compassion and dedication that defines our Street Medicine Program. The patient, who had been unhoused with his beloved dogs for over a year following a house fire, faced a critical barrier: he had nowhere to recover post-surgery and no one to care for his companions. Understanding that quality healthcare extends beyond medical procedures, Eric coordinated with our VHP Street Medicine team to arrange temporary housing through our respite program. When the patient developed post-operative complications requiring overnight hospitalization, Eric went far beyond his clinical duties—he stayed overnight at the respite facility to personally care for the patient’s dogs. For over 36 hours, Eric remained at the respite center, ensuring continuity of care while sacrificing time with his own family, including his wife, six-month-old baby, and dog. His actions enabled a surgery that would have otherwise been impossible, allowing the patient to maintain his independence and continue the physical activities essential to his survival and wellbeing. Eric’s commitment exemplifies our organization’s mission to provide whole-person care regardless of circumstances. His compassion, clinical expertise, and unwavering dedication to patient advocacy make him an invaluable member of our street medicine team and a true champion for our community’s most vulnerable members. Art from the Heart is a collaborative partnership with the Allentown Art Museum, created from a shared passion to empower families through the transformative power of art and expression. This free quarterly program is open to all VHP Children’s Clinic pediatric patients and their families and was designed to help participants process emotions through creativity and mindfulness. Margie Makowski, Manager of Accessibility at Allentown Art Museum, meticulously crafts each session, introducing new mediums and coordinating with museum exhibits. “Each program feels like a warm reunion, as we often see returning families and new VHP families take part in artmaking during the program. This partnership has community and wellness at its core—offering a space for families to create together and focus on themes such as mindfulness, self-expression, and connection” said Makowski. Since its launch two years ago, Art from the Heart has grown significantly—expanding from just 10 participating families to now welcoming 30 families each session. As both new and returning families continue to engage, Art from the Heart stands as a powerful example of what’s possible when creativity and healthcare unite for community impact. I cannot speak highly enough about the impact of this program on our kindergarten students. Our students proudly wore their new glasses for weeks, eagerly showing them to anyone who visited their classrooms. They receive a full exam from trained professionals right in their school, and glasses if needed at no cost. This pilot program, which ran October to May, was limited to students identified by the school nurse through vision screenings as needing further evaluation by an eye care specialist.

Valley Health Partners joined forces with United Way of the Greater Lehigh Valley, Easton Area Neighborhood Center, and Sodexo to host an interactive cooking demonstration at Easton Public Market. More than just a lesson in healthy, affordable meals, this pilot program brought generations together to cook, share stories, and enjoy a communal dining experience. Attendees even took home meal kits to recreate the dish, reinforcing food security and healthy eating habits. Together, we are building community—one plate at a time. PARTNERSHIP SPOTLIGHT SUMMER READING SERIES
VHP Children’s Clinic launched its annual Summer Reading Series, a seasonal initiative designed to promote literacy, foster curiosity, and build stronger futures for children in the community. Now in its fourth year, the series was created by Tonya Kemp, RN, a Case Manager at VHP Children’s Clinic who led the effort with the belief that every child should have access to books. This year’s theme, Planting Seeds That Grow, celebrates learning, growth, and community. In collaboration with the Boys & Girls Club of Allentown and the Kellyn Foundation, the 2025 series kicked off in June with a hands-on program centered around creating community through gardening. The event featured a special reading by Boys & Girls Club CEO, Katarah Jordan, who shared her children’s book It’s Harvest Time , a story that teaches the value of family, teamwork, and the joy of sharing through the lens of gardening. Children took part in interactive gardening activities led by the Kellyn Foundation, selected free books and bookmarks to take home, and entered a raffle for the chance to win scooters, making for a day filled with learning, fun, and community connection. VHP’s Summer Reading Series has distributed more than 10,000 free books to pediatric patients across the Lehigh Valley.

The first-ever Fiesta on Hamilton made its grand debut in May, delivering an unforgettable celebration of Latin culture. Presented by Valley Health Partners, the Lehigh Valley Chamber of Commerce, the City of Allentown, and MEGA 101.7 FM, this brand-new, free event took over three blocks of Hamilton Street in Allentown. Fiesta on Hamilton brought world-class Latin music and electrifying performances to the heart of Allentown, paired with some of the region’s best food vendors serving up delicious flavors from across Latin America. The event also featured a dedicated health-focused block aimed at improving community wellness. Attendees explored interactive games, exciting giveaways, and a wide array of health and wellness resources designed to inspire healthier living. Valley Health Partners Community Health Fair was made possible through the generous support of Jefferson Health Plans, Lehigh Valley Health Network, City Center Group, and Embassy Bank for the Lehigh Valley. Fiesta on Hamilton is set to become a new annual tradition, uniting the community in a spectacular celebration of Latin culture.
