Valley Health Partners 2024-2025 Impact Report

ABOUT COMMUNITY HEALTH CENTERS For 60 years, Community Health Centers have delivered comprehensive, high-quality care to communities nationwide. Rooted in the neighborhoods they serve, health centers go beyond medical care—expanding access, promoting health equity, lowering health care costs, and making a lasting impact on the well-being of their communities. Health centers play a vital role in expanding access to essential primary care by removing common barriers like cost, lack of insurance, transportation challenges, and language differences. In doing so, they deliver meaningful benefits to both patients and the broader health care system.

OUR HISTORY The clinical roots of VHP can be traced back to several decades ago, when it was part of Lehigh Valley Health Network (LVHN), now part of Jefferson Health. In 2020, VHP transitioned into an independent, separate non-profit entity, focusing solely on primary and preventative care. In 2021, VHP was successfully designated as a Federally Qualified Health Center Look-Alike (FQHC-LA), commonly known as a Community Health Center. While Federally Qualified Health Centers (FQHC) receive federal funding, FQHC-LA’s, like Valley Health Partners, do not.
