Completed Business Application Personal Financial Statement Personal Tax Returns (previous 3 years for all guarantors) Current Paystubs Business Plan, if applicable Business Tax Returns (previous 3 years) Year-to-Date Profit & Loss Statement, Balance Sheet, and current Debt Schedule for business

For Construction Loans Purchase Contract

For Auto/Equipment Loans Bill of Sale/Dealer Sheet

For Real Estate Loans Purchase Contract

Cost Breakdown Draw Schedule Building Plans Material Listing Insurance Quote

Copy of Title (if private seller) Verification of insurance or Insurance quote

Monthly rent charged $ Deed/Legal Description Insurance Declarations page or quote Real estate tax information
