30%+ FTE EMPLOYEES

PLAN PAYS 60% OF EMPLOYEE AVERAGE MONTHLY PRE-DISABILITY EARNINGS* UP TO A MAXIMUM OF: o $15,000/MONTH FOR PHYSICIANS o $10,000/MONTH FOR NON-PHYSICIAN PROVIDERS AND STAFF YOU MUST BE DISABLED FOR 90 DAYS BEFORE THE BENEFIT PAYS AS AN ELIGIBLE EMPLOYEE, YOU ARE AUTOMATICALLY ENROLLED IN THIS BENEFIT DISABILITY DEFINED AS SPECIALITY/SUBSPECIALITY *PRE-DISABILITY EARNINGS ARE DEFINED AS AVERAGE MONTHLY EARNINGS FROM “ALL SOURCES OF INCOME”, INCLUDING BASE AND VARIABLE PAY, IN EFFECT FOR THE 1) 12 CALENDAR MONTHS IMMEDIATELY PRIOR TO THE LAST DAY YOU WERE ACTIVELY AT WORK BEFORE YOU BECOME DISABLED; OR 2) THE TOTAL NUMBER OF CALENDAR MONTHS YOU WORKED FOR THE EMPLOYER AS AN ACTIVE EMPLOYEE, IF LESS THAN THE ABOVE PERIOD.
