Message from your CNO
Greetings!
I am Neena Philip, Chief Nursing Officer at NYC Health and Hospital North Central Bronx. I am excited to an- nounce that North Central Bronx is embarking on our Path- ways to Excellence journey. It is a momentous occasion where we are laying the foundations, creating means and ways to transform our nursing culture for excellence. The Pathway to Excellence is a prestigious American Nurses Credentialing Center designation. Our nursing team at eve- ry level and every department will work together to lead the journey and create a vibrant, healthy, and innovative
environment. We are cultivating a culture where our teams love to come to work because they are accountable, engaged, and empowered for better patient and staff outcomes. Studies show that patients receive high - quality and safe care when nurses are empowered, happy, and educated. The tenets of the Pathway to Excellence are Shared decision - making, Leadership, Safety, Quality, Well Being, and Professional Development.
What does it mean when we talk about Shared Decision making? It ’ s where Nurses are involved in nursing policy, practice, and procedure decisions.
Leadership - We cultivate leadership capabilities among our teams to support, mentor, educate, and lead our nurses to excellence.
Quality - Nurses are engaged in evidence - based practice solutions to improve the quality and safety of care.
Safety & Wellbeing - We believe our strength comes from our workforce. We can achieve excellence when we protect the safety and ensure the well - being of our staff. Professional Development - Nurses adopt a quest for lifelong learning through onboarding, certifications, and continued professional and academic education. We will need everyone ’ s active participation in this journey. This takes hard work and not settling in for the status quo. However, we look forward to this journey that will be transformative in the lives of our nurses, our patients, and this organization.

General Nursing Orientation & Pathway to Excellence Journey
New nursing employees at North Central Bronx Hospital attend the NYC Health + Hospitals General Nursing Orientation every month. There are many new nursing recruits at NCB, including staff nurses, APNs, DONs, ADNs, PCTs, PCAs, and BHAs, as well as clerical staff. Upon completion of the Nursing Education Orientation, new hires will be released to their unit for unit - based orientation. System orientation for new employees, systems orientation for support staff, systems orientation for nursing, and EPIC train- ing courses are part of the didactic and virtual learning activities. Nurses are validated during orientation for their skills and competencies rel- evant to their job descriptions. The nursing education department has dedi- cated educators who cater to the initial needs of the orientees and are instru- mental in carving the transitional pathway to NCB.

Optimizing Nursing Workflow
The use of technology to improve user workflow is one of the most out- standing achievements in nursing informatics. We live in a technologically advanced age where information can be sent and received with a click or a touch of a button. North Central Bronx Hospital has introduced the Connex Spot Vitals Machine. This machine allows the end user to login as a provid- er using their TKID located on their ID badge. Once the user is signed in the user is able to scan the patient ’ s ID. Vitals are taken by the user includ- ing blood pressure, heart rate, temperature, and oxygen saturation. Respi- rations, height, weight, and pain indicators can be manually entered into the device. After the vitals are obtained in the machine the end users
saves/send the vitals. The device sends the vitals of the patient into the vitals flowsheet within the patient ’ s EPIC chart. Previously the user would have to transcribe vitals manually into the patient ’ s chart in EPIC. Our end users in the Medical Surgical service line have expressed that the Spot Vital device is a useful tool that improved their work- flow for obtaining vitals. I look forward to future technology that will help automate workflow in Nursing.

Medication Strategies: Protect your patient and your license
The seatbelt alarm can be annoying. There is no voice like "Alexa" that re- peats, "Seat Belt, please." It serves as a gentle reminder that safety is para- mount because the seat belt protects us against adverse events. I realized how important it is to protect our patients, our licenses, and our emotional wellbeing. Despite the best efforts to prevent medication errors, many systems can fail. This was the case in a recent ISPM article when a pharmacist made a mistake and scheduled an antibiotic to be given starting next year. Despite the fact that the omission did not contribute to the patient's condition, the article leaves us wondering about how efficient the systems designed to assist us are.
It is essential to develop habits that make administering medications sec- ond nature. It should be routine and habitual to double - check medications.
The administration of medications requires basic knowledge, skills, and the incorporation of medication administra- tion rights. There are many interruptions, distractions, and challenges nurses face every day. It is very important for us to develop safe behaviors that will lead to best practices for the safety of our patients and our mental health. It is important to remember that the medication order and the EHR medication scanning system contain the five rights of medication administration: the right patient, the right medication, the correct dose, the right route, and the right time. This list of five rights should be reviewed before administering drugs, as medication errors have the po- tential to cause fatal injuries. The ISMP calls these "Independent Double Checks" In the June 2019 newsletter, and the ISMP recommends these play an essential part in medication safety. An independent double - check is conducted by a pharmacist and a regis- tered nurse to ensure the medication was prescribed, administered, routed, and timed correctly. The five rights emphasize the importance of the right reason and proper education. These and other techniques have always helped me in my years of practice, though they are not specifically mentioned. First, we must familiarize our- selves with the medications specific to the unit in our current specialty. Critical thinking skills are needed to deter- mine the right reason for medication administration, including knowledge of the reason/indications, dose, and side effects that are relevant to the patient's current diagnosis and complaint. Patient education is an important compo- nent of proper education. Additionally, patients must understand why they are receiving medication as well as the risks associated with it. Edu- cating the patient is our best line of defense in providing safe care. Behaviors involving incorporating the rights of Medication Administration and double - checking are safe behaviors. Safe Behaviors are our best defense to protect our patients, our licenses, and our mental health. The extra time we spend performing these practices will never equal the time, energy, and thinking that comes after a catastrophic/fatal medication error. Just like that annoying alarm that comes on when we drive without the seat belt, that same alarm should come on during medication admin- istration when we do not follow those double checks that are safe behaviors. These are especially important when administering medications that cause the most significant harm to the patient. We must keep in mind that a single medication error could be catastrophic.

Showcasing the Science: CAUTI Poster Presentation
In June 2022, NYU Langone Health sponsored its 25th Annual Nursing Re- search Conference. The North Central Bronx (NCB) Hospital Nursing Educa- tion team submitted a poster entitled "PREVENTION OF CAUTI (Zero Harm): ENHANCING CLINICAL COMPETENCY OF NURSING STAFF." The team comprised of the Director Mario Bustamante, Senior Educator Per- sha Gayle, Staff Nurse Mariet Duporte - Arthurton, and Assistant Coordinating Manager Kyle Massab, who were there as presenters. Several questions were raised regarding the buddy system implemented at all NYC Health and Hospi- tal facilities that were highly compliant. The team identified an enhanced competency in preventing CAUTIs through a practical approach. The nursing staff at NCB Hospital have acquired a greater understanding and skill concern-
ing caring for patients with indwelling Foley catheters due to the program. To ensure nursing staff is given protective time to participate in simulation training, nursing leadership and high - level management must support it. It is crucial to set up a planned and organized schedule so that the Unit Leaders have enough time to register their staff. For the best practices to prevent CAUTI to remain effective, annual compe- tency - based validation is essential, as well as continuous surveillance for quality and performance improvement. Bernadette Mazurek Melnyk - Ph.D., APRN - CNP, FAANP, FNAP, FAAN, a nationally and globally recognized nurse scientist for her clinical knowledge and innovative approaches to healthcare challenges, was the speaker at this event. Dr. Melnyk delivered the keynote address entitled "Achieving Healthcare's Quadruple Aim with Evidence - Based Practice, Research, and Clinical Wellbeing: Key Tactics for Success." As a result, the importance of nursing and research was further amplified.
