Appendix A: Assistive Technology Assessment – Low Incidence Referral

Upon completion of the assessment, a report will be sent to the Director of Special Education by either the agency that conducted the assessment or the Coordinator, Transition Services at the DMSELPA, with a request to schedule an IEP meeting. The Director of Special Education should reconvene the Individualized Education program (IEP) team to review the report finding and recommendations of the assessment and determine whether or not assistive technology services are warranted.

If the IEP team agrees that services are not needed, it will be documented in the IEP notes.
