BP 5004 – Instructional Planning and the IEP

give an overview of the IEP meeting process, including the document, DMSELPA form 68A, which describes the student’s program .

The DMSELPA IEP is used by all Local Education Agencies (LEAs) within the SELPA and includes all required components.

All special education and related services determined by the IEP team to be necessary for a student to benefit from education shall be listed on the IEP. The IEP shall include the date of initiation and frequency of service. Each participating LEA shall assure that each student with a disability is provided services in accordance with his/her IEP, regardless of which agency or contractor provides the service. According to California Education Code § 56326 , a child can be referred for further assessment by the California School for the Deaf or Blind, or the Diagnostic Centers. This assessment, however, does not constitute placement in the state special schools. This information, along with other relevant factors, would assist the IEP team in their decision-making process regarding the most appropriate placement for the child.
