SELPA Referral Procedures
• Most current complete annual or triennial IEP
• Psycho-educational evaluation dated within three years from the time the student was identified as eligible for special education and related services • All current assessment reports, including but not limited to, psycho-educational evaluations, functional behavioral assessment, speech assessments, occupational therapy assessments
Pertinent medical documentation

• Any additional information in support of the referral

Timeline for Assessment
• Upon receipt of the completed referral:
• The SELPA school psychologist who coordinates the RTC placements will forward the referral to the DMCC for processing.
• SELPA will coordinate with the DMCC to develop an assessment plan and send to the parent/guardian for informed consent to initiate the evaluation.
• The DMCC will send out an Assessment Plan (DM 66) to the parent/guardian for consent for assessment.
• Upon receipt of the signed Assessment Plan (DM 66), the 60-day timeline begins.
• The DMCC will conduct the assessment.
• The DMCC will contact the SELPA school psychologist who coordinates RTC placements when the assessment is complete to schedule an IEP.
4.
Timeline for Services
Upon completion of the residential assessment, the SELPA school psychologist who coordinates RTC placements will coordinate an IEP team meeting to determine eligibility for residential placement services.
If the IEP team agrees that the student is eligible for and requires residential placement as the least restrictive environment, the DMCC will
