Last Day In Hospital

The discharge nurse took a moment to explain all the steps needed while recovering at home during her leave of absence from work.

In a fact-filled, accelerated ( the wheelchair was waiting! ) discharge planning session, they reviewed: pain management, wound care/caring for the bandage (dressing) over the incision, caring for a surgical drain, stitches and staples, recognizing signs of infection and lymphedema, hygiene, and the importance of resting. Rachel asked the nurse questions related to recovery but felt overwhelmed with all of the information that was provided to her. The nurse noticed her body language and reassured her. Furthermore, the nurse introduced her to the Concierge Care (BehavioralRx) program that will guide her to best manage toward full recovery upon discharge.

Rachel completed the initial Concierge Care assessment to be used to personalize the engagement experience that fits with her specific lifestyle, emotional and physical challenges and preferences. Some of her input included:

1. Physical Activities: “I always wanted to try yoga.” 2. What time of day is the most difficult for you? “Anxiety strongest in evening”; “ Trouble sleeping” 3. What time of day do you typically eat breakfast, lunch and dinner? “10 a.m., 2 p.m. and 5:30 p.m."

4. What time do you get ready for bed? “9 p.m.” 5. Do you take oral medication for treatment? “Yes” 6. What kind of oral medication do you take? “ IBRANCE” 7. When do you normally take your medication? “Dinner” 8. Do you ever forget to take your medication? “Yes”
