Conclusion: Evolution and Monaco demonstrated comparable geometric accuracy, which may partly have been influenced by the clinical GTV having originated from the Monaco-based GTV. Nevertheless, radiation oncologists consistently favored Evolution for its smoother contours. Results: Median DSC was 0.85 for Evolution (IQR 0.11, range 0.31) and 0.87 for Monaco (IQR 0.09, range 0.35). Median absolute HD was 6.46 mm for Evolution (IQR 3.62, range 7.89) and 5.62 mm for Monaco (IQR 2.47, range 7.97), and median MDA was 1.02 mm for Evolution (IQR 0.46, range 2.51) and 0.87 mm for Monaco (IQR 0.40, range 2.60) (Figure 2). Although geometric metrics were comparable, three radiation oncologists consistently preferred Evolution contours, selecting them in 78.6% cases versus 16.7% for Monaco, with 4.7% showing no clear preference. Notably, in 64% of fractions, all observers unanimously favored Evolution, often commenting that these contours appeared smoother, whereas unanimous preference for Monaco was not observed.
