Conclusion: This study introduces the first delivery-efficient 4pi optimization algorithm to improve the dosimetric performance of skull-based chordoma proton beam therapy planning. With the extra degree of freedom, SPArc-4pi can significantly enhance coverage of the GTV while sparing the OARs abutting the target, which we expect will translate to improved local control in complex cases. The delivery time using 4pi arc trajectory was also superior to IMPT, supporting its feasibility and promise for future clinical implementation. References: 1. Saito, T. et al.
