encounters to ensure consistency and completeness. All extracted results were independently assessed by two reviewers, and any discrepancies were adjudicated by a third reviewer. Results: A total of 25 encounters of 14 prostate cancer patients were recorded (mean duration of recording: 13.40 ± 9.23 minutes) and transcribed (mean words in transcript: 2207 ± 1646), then processed through the pipeline. From the initial 66 extracted toxicities, 5 were excluded because they were unrelated to the radiotherapy treatment (Table 1).
No additional toxicities were missed by the model. Cohen’s Kappa was 0.79, showing substantial agreement, specificity was 93.7% and F1 score (weighted) was 0.92. Conclusion: This automated pipeline accurately extracted and graded CTCAE toxicities from clinical encounter recordings, showing strong agreement with manual review. Further studies are warranted to validate its performance across other disease sites and larger patient cohorts. Among the remaining events, 57 (93.4%) toxicities matched those reported by the manual annotators, while 3 (4.9%) showed discrepancies in grade assignment and one (1.6%) was incorrect (Figure 1).
