They can provide the required supervision in online adaptive radiotherapy, bridging the responsibilities of radiation oncologists and medical physics experts, and oversee effective implementation and quality assurance. In proton therapy, technical physicians contribute to complex treatment plans and have a distinct, clinical role in the treatment for specific indications, e.g. uveal melanomas. In the field of hyperthermia, technical physicians work on treatment strategies, monitor high- quality therapies, and are actively involved in the outpatient clinic. Moreover, technical physicians automate workflows through AI and scripting [3].

Material/Methods: Two anonymized clinical cases—a T1 vertebral metastasis and a centrally located lung adenocarcinoma—were distributed to all Belgian RT departments with consensus contours, target prescriptions, and organ at risk (OAR) constraints. Required dose prescription was 35 Gy in 5 fractions for the spine case and 60 Gy in 8 fractions for the lung case. Each participating centre was asked to perform the treatment planning according to the local practice, while respecting the provided OAR dose constraints. Planning objective were defined to ensure PTV D95 = 100% prescribed dose (Dp). DVH data were extracted for target volumes and OARs (spinal cord, brachial plexus, esophagus, trachea). Results: Twelve centres submitted their planning information for central evaluation. All plans used 6MV Flattening
Conclusion: The Dutch experience demonstrates that technical physicians can successfully address the need for healthcare professionals with combined medical and technological expertise Their integration into radiotherapy departments supports safe and effective implementation of technological innovations in radiotherapy clinics, despite current constraints in resources.
