A systematic search was conducted in PubMed and Scopus for English-language articles published between January 2015 and September 2025, following PRISMA-ScR methodology. Search terms included “prostate cancer,” “radiotherapy,” “mindfulness,” “meditation,” “stress reduction,” and “relaxation.” Eligible studies investigated mindfulness-based or acceptance-based interventions in adult males undergoing or recently completing prostate RT, and reported psychological, physiological, or quality-of-life outcomes. Screening and data extraction were performed independently by two reviewers using Rayyan. Extracted data included study design, intervention characteristics, timing relative to RT, and key outcome domains. The protocol was registered on the Open Science Framework https://doi.org/10.17605/OSF.IO/CW5JY Results: From 109 records screened, five studies met inclusion criteria: two randomized controlled trials, one prospective cohort, one mixed-method pilot, and one systematic review with meta-analysis. Feasibility: All interventions were safe, brief, and well tolerated. The daily 5-minute audio-guided mindfulness protocol integrated into RT workflow achieved >75% participation and >85% retention. Psychological outcomes: All studies reported reductions in anxiety and perceived stress, with improved emotional regulation and self-efficacy. Physiological measures: Significant post-session reductions in heart rate and muscle tension were documented, indicating acute relaxation effects. Quality of life: Meta-analytic data showed small-to-moderate improvements in fatigue and overall well-being, with maintained benefits at short follow-up. Extended effects: One mixed-method study described enhanced sexual intimacy and relationship adjustment after mindfulness-based group therapy in survivors. Overall, MBIs demonstrated consistent feasibility and positive impact across diverse formats and delivery models, particularly when implemented during treatment Conclusion: Mindfulness-based interventions are feasible, safe, and beneficial for men undergoing prostate RT. Integrating short, structured mindfulness sessions— especially audio-guided formats delivered before or during treatment—may improve stress management, enhance emotional resilience, and support treatment adherence without disrupting workflow. Further multicenter randomized studies are warranted to standardize protocols, validate physiological endpoints, and assess implementation in routine radiotherapy practice.
