oncologists, medical physics experts, radiation therapists, and quality managers, developed an initial list of QIs based on a previously conducted comprehensive literature review. This list was distributed to experts from each National Society for evaluation through a Delphi process. QI were rated across four parameters: importance, scientific validity, benchmarking utility, and ease of data collection and those with the highest score were prioritized for further consideration. Despite high scores, QIs that were site-specific or closely linked to the capabilities and workload of individual Radiation Oncology departments were excluded to ensure universal applicability. The multidisciplinary panel refined the top ranked QIs into first-level binary QIs to facilitate benchmarking at European level. Some selected items were identified as potential second-level quantitative QIs for centers capable of providing detailed data. These indicators can be stratified by treatment site, as different sites may require distinct benchmark standards. Results: The core group identified 114 potential QIs from the literature review, which were streamlined to 37 Qis, following the Delphi process. Each QI is summarised in Table 1, and a clear description is provided.Of these, 24 were classified as core QIs. For 10 QIs, additional quantitative metrics were proposed to enable benchmarking, where data availability allows. Table 1. Minimum set of radiotherapy-specific Quality Indicators. Conclusion: A consensus-based, minimal, and non-site-specific set of Qis, tailored to the European context, has been proposed using standardised terminology and clear guidance. The next step is to pilot the set of QIs in selected Radiation Oncology departments across Europe to evaluate its applicability for self-auditing, multicentric data collection, and benchmarking. Harmonising QIs across Europe remains essential to promote consistent, high-quality, and equitable radiotherapy care. Keywords: quality indicators, delphi process
