Purpose/Objective: The recurrence patterns in patients diagnosed with stage III Locally Advanced Breast Cancer (LABC) who undergo mastectomy followed by post-mastectomy radiotherapy (PMRT) are not well understood. Gaining insight into these patterns is crucial for refining treatment strategies and enhancing patient outcomes. This study aims to analyze the recurrence rates and patterns in this specific patient population, while also investigating potential differences between those who underwent modified radical or simple mastectomy (M) and those who received subcutaneous mastectomy with reconstruction (SMR). Material/Methods: A retrospective analysis was performed utilizing case series from a single institution. This study included patients diagnosed with stage III LABC who underwent mastectomy, received PMRT using either 3D conformal radiotherapy (3D-CRT) or intensity-modulated radiotherapy (IMRT), and maintained a minimum follow-up period of six months. To evaluate the relationship between treatment outcomes, recurrence, and the type of mastectomy, we employed Kaplan- Meier survival analysis along with Chi-square and log- rank tests. Results: A total of 86 patients diagnosed between 2015 and 2024 were evaluated. Median age was 51 (Range: 26- 80). The median follow-up was approximately 58 months (Range: 10-122 months). Out of these patients, 19 (21.1%) had clinical T4 disease, and 8 (9.3%) had clinical N3 disease. Among the patients, 71 (82.6%) underwent M, while 15 (17.4%) had a SMR. Patients' characteristics are summarized in Table 1. During the follow-up period, recurrence was observed in 13 (15.1%) patients, including local recurrence (LR) in 1 (1.2%) patient, regional recurrence (RR) in 2 (2.3%) patients, and distant metastases (DM) in 13 (15.1%) patients. The characteristics of patients with distant metastases are presented in Table 2. Although the recurrence rates were numerically higher in the mastectomy group, there was no statistically significant difference in recurrence rates and patterns between the two groups (p > 0.05).
Conclusion: Our study is the first to investigate recurrence patterns in patients with stage III LABC following PMRT. Distant metastases are the primary recurrence pattern in locally advanced breast cancer after mastectomy and PMRT. Both M and SMR show similar recurrence patterns.
