phase I followed by rIMRT in phase II. Scoring was carried out for each of the two phases using five PTV matrices (D95, CI, HI, GI, CN) and four OAR parameters (D2 for bladder and rectum, Dmax for right and left femoral head (Figure). Plans with a non-significant mean difference from the ideal PTV and OARs matrices were scored 0, while those with significant differences were scored (1, 2, or 3) based on the value of mean difference. A final rank was obtained for cumulative phases I and II, considering both PTV and OAR parameters, with the lowest score being the best plan and vice versa.
Conclusion: Given the progressive regression of GTV over 5 weeks of pre-brachytherapy EBRT, adaptive planning with AIMRT or HART appears to be the best option compared to sIMRT or s3D- CRT. This may be useful as an addendum to the current EBRT guidelines in LACC1. References: 1. Results: 170 plans in 34 patients, stages IIB-IVB were analysed. GTVreduced from236 ± 114 cc(pre- EBRT) to136 ± 72.5 ccat re-CECT (p<0.001). For the PTV metrics in phase I, 3D-CRT and IMRT scored 8 and 5, respectively, while 3D- CRT and IMRT scored 8 and 4, respectively, for OARs. Inphase II, the five PTVmetrics across three rival plans — s3DCRT, sIMRT, and rIMRT were scored as 11, 10, and 6, respectively. For OARs, the scores for s3D- CRT, sIMRT, and rIMRT were 5, 9, and 4, respectively.Thus, the final scores ranked these plans in favour of adaptive strategies, with AIMRT, HART, sIMRT, and s3D-CRT in descending order (Table).
