While strong evidence demonstrated the role of intensified chemotherapy (CHT), few data documented the effect of intensified radiotherapy (RT) on survival. Conclusion: In TNT with consolidation chemotherapy for LARC (UICC II/III), CR was similar between women and men, whereas acute high-grade toxicity, notably gastrointestinal and haematologic, was consistently higher in women. No differences in short-term survival were observed. These findings support a toxicity risk assessment that considers sex differences, as well as a prospective evaluation of tailored supportive care, dose modulation, and monitoring, all of which can be achieved while maintaining comparable tumour control.
