CI (7.8%, Inf). Pre-specified adjusted multivariable logistic regression with imputed data produced an odds ratio 2.27 (80% one-sided CI (1.54, inf), one-sided p-value 0.038), statistically significant at the one-sided 20% level. Pre-defined sensitivity analyses including cCR data outside the +/- 5-week window (15 (41.7%) SDRT vs 46 (59.7%) EDRT) and a modified cCR allowing superficial mucosal ulceration (13 (36.1%) SDRT vs 44 (57.1%) EDRT) showed similar significant results.

Conclusion: EDRT demonstrated a statistically significant and clinical meaningful increase in 6-month cCR rate, representing the first randomised report showing early benefit of external beam dose escalation in early rectal cancer. Keywords: Rectal cancer, organ preservation, dose escalation
