6.2.3 Part-Time Employees - Dental insurance premiums referred to in paragraph 6.2.1 of this Article shall be provided by the District on a pro rata basis for members of the bargaining unit whose assignment is less than full-time. The pro rata amount shall be determined by multiplying the employee's full-time equivalent employee ratio by the amount of dental insurance allocation provided for in paragraph 6.2.1 of this Article. The balance of the premium must be paid by the

employee by payroll deduction.
6.3 Group Life Insurance - The District shall provide no less than a $50,000 life 8 insurance policy for members of the bargaining unit employed twenty (20) hours or more 9 per week effective January 1, 2017. The specific carrier and amount shall be determined 10 by the Shared Decision Making Benefits Committee. The member shall have the option of 11 purchasing additional insurance coverage by payroll deduction. 12 6.4 Allocated Dollar Amount - The allocated dollar amount for the Shared Decision 13 Making/Benefits Committee to designate shall be $10,352 per full time equivalent 14 employee. This amount will be allocated for health, dental, life and if sufficient funds 15 remain for vision, wellness or other employee benefits. If during the term of this agreement, 16 any across the board general benefit level increase is approved and granted by the Board 17 of Education to any other employees in the District, the level of increase shall also be 18 granted at that same rate to Association bargaining unit members. 19 6.5 Annual Enrollment Requirements - All bargaining unit members must select 20 benefit options on the district provided form during the annual open-enrollment period. 21 6.5.1 – Failure to Select Plan – Members of the bargaining unit who fail to make 22 a selection during open enrollment shall be re-enrolled in their current plan or continue to 23 decline their benefit option as applicable. Bargaining unit members who opt out of 24 enrolling in any District-approved group health plan will not be eligible for any benefits 25 outlined in paragraph 6.1 of Article VI. 26
