TAX INFORMATION: IMPORTANT HEALTH COVERAGE TAX DOCUMENT (1095-C FORM) According to new Federal requirements, you may now access your Form 1095 C electronically. Brunswick Community College is no longer required to automatically mail you a 1095-C. Accessing your Form 1095 is quick and easy. Simply visit the North Carolina State Health Plan website to download the form. Here are specific instructions: • Log into your State Health Plan member benefits portal using the same login credentials used for the enrollment system during open enrollment. • At the top of the page, hover over Benefits, then select 1095 Tax Statement. • Select the tax year document you are looking for, then download to print or save.
If you prefer that we mail a copy of your Form 1095-C, please reach out BCC Human Resources (see below). You will receive your form no later than 30 days after we receive your request.
If you are no longer employed by Brunswick Community College, or you retired in 2025, you will receive your Form 1095-C in the mail.
