Complete the Incident Report on the intranet. Provide recommendations for corrective actions. Indicate the need for additional or remedial safety training. Employees shall cooperate with the investigation process.
9. Employees shall be truthful and thorough during the investigation process. If the employee fails to do so, the employee may be subject to disciplinary action, up to and including termination. 10. Employees shall not discuss or comment on the accident/incident to other non-involved parties.

On-the-job Injury Procedures The following procedures should be followed when an injury on-the-job occurs: 1. When an accident occurs, the employee must report the accident to their supervisor immediately. In the absence of the direct supervisor, inform the Department Director, Human Resources, or an Assistant Village Manager. 2. The supervisor will inform Human Resources of the need for medical treatment. If the injury is perceived to be life threatening, the employee should immediately be taken to the nearest emergency room and Human Resources notified as soon as is reasonably possible. The emergency room staff MUST be informed that it is a workers’ compensation claim for the Village of Pinehurst Municipal Government. 3. Employees should obtain the proper treatment authorization forms from Human Resources if the injury is not life-threatening. 4. The supervisor must complete the Employee Work Injury Report on the intranet within 24 hours of the injury whether or not first-aid or medical treatment is necessary. 5. All injuries are to be investigated by the supervisor as soon as possible following the accident. A summary of the accident and investigation are to be included on Employee Work Injury Report and forwarded to Human Resources immediately upon completion. A Form 19 (or other process deemed appropriate by the third-party administrator) will be completed by Human Resources and sent to the employee and the third-party administrator. If the employee does not agree with the description or time of the accident given on the Form 19, the employee should make a written report to Human Resources within thirty (30) days of the injury. A follow-up investigation will be conducted. Employees are also permitted to file a form 18 W/C claim with the Industrial Commission with their statement(s) of fact. 6. If the injured employee is placed on a transitional work assignment or given work restrictions by the authorized medical staff, the employee in consultation with the supervisor must then contact Human Resources for specific return to work instructions.
