7. An employee must report to their Department Director and Human Resources after each doctor’s visit and provide a medical release to Human Resources prior to returning to work. 8. If medical treatment is required or prescriptions are issued, the employee must notify the treating authority that it is a Workers’ Compensation Claim. EMPLOYEES SHOULD NOT USE THEIR PERSONAL MEDICAL INSURANCE CARD. **Additional details regarding on-the-job injuries are located in the Workers’ Compensation Policy. The Village expects each employee to follow and support the safety program. If infractions occur, it is important that procedures be in place for both the Village and the employee to resolve such issues. Accident/Incident investigation reports must be submitted to Human Resources and/or Risk Management within 24 hours of the accident when possible. Failure to report any and all injuries or vehicle accidents, regardless of how minor, may result in corrective action up to and including termination.
