ABOUT THIS GUIDE This guide describes the benefit plans and policies available to you as an employee of the company. The details of these plans and policies are contained in the official plan and policy documents, including some insurance contracts. This guide is meant only to cover the major points of each plan or policy. It doesn’t contain all of the details that are included in the Summary Plan Descriptions (SPD). You can find all Plans’ SPDs on the Akima Benefits Center Portal at www.akimabenefits.com . If there is a conflict between this guide and the official Plan Documents, the Plan Documents will govern. The company reserves the right to amend, modify, or terminate its plans and change costs at any time and without prior notice. Your participation in any of the company’s Benefits Plans does not give you the right to continued or future employment. Summary of Benefits and Coverage (SBC) Health Care Reform requires Medical plans to provide participants with a Summary of Benefits and Coverage (SBC), a document that summarizes, in an easy-to-read format, important information about Medical plans for which you may be eligible, including any deductibles, out-of-pocket maximums, and covered services. Your Medical plan SBC is included in this benefits enrollment pack. If you have Medicare or will become eligible for Medicare in the next 12 months, a new federal law gives you more choices about your prescription drug coverage, starting in 2017. For more details, please reference the Legal Notices located in the back of this guide.
