Last year, 416 corrugated facilities were recognized for plant safety. What is the OSHA 300 Log? The OSHA Form 300 is a form for employers to record all reportable injuries and illnesses that occur in the work- place. Where and when they happen, the nature of the case, the name and job title of the team member injured or made sick must be included, as well as the number of days away from work or on restricted or light duty. Establishments with 250 or more employees (at any time during the reporting year) must submit OSHA Forms 300 and 301 until January 2019 electronically. However, you must keep the forms on file for five years as OSHA will continue to obtain the records through inspections and enforcement actions.
Reporting Reminder and Facility Recognition Opportunity Right now is the perfect time to remind you of a new year to-do that involves reporting - specif- ically, OSHA Injury and Illness data reporting. If you are responsible for health and safety in your compa- ny, you probably have some part in keeping up an OSHA 300 log. A summary of those work-related inju- ries and illnesses must be submitted in your OSHA 300A form for the calendar year 2021 by March 2, 2022. You must document all work-related fa- talities and reportable injuries and illnesses in the OSHA 300 Log. Establishments are required to record the above if they result in the following: • Death • Days away from work • Work restrictions or job transfers • Medical treatment beyond plant first aid • Loss of consciousness • Diagnosis of a significant injury or illness There are exceptions, and doing some digging will help determine if an incident at work resulted from a non-work-related ex- posure or event. Form 301 Incident Report The Injury and Illness Incident Report Form 301 records a case-by-case occur- rence of an injury or illness. You have seven days to fill out Form 301 or an equivalent when a recordable work-related injury or illness has occurred. Some state workers’ compensation, insurance, or other reports may be acceptable. Where to find OSHA Forms More details on the OSHA standard are available in 29 cfr §1904. Use the following path for the PDFs of Forms 300, 300A, and 301, also available in a fillable format. To complete Form 300A, you’ll need your Em- ployee Identification Number (EIN). Create an Injury Tracking Application (ITA) account and follow the instructions.
