5.1

Attendance and Punctuality Punctuality and daily attendance are important for many reasons at COPC. Although every employee should strive for a perfect attendance record, we understand there may be occasions when an absence is unavoidable. Employees are required to report any absence from work to their Direct Supervisor, which, in most cases, should occur no later than one-hour before the scheduled shift/workday. Site-specific and department- specific notification procedures should be followed when directed to do so by management. When an employee is sick for three or more days in a row, a doctor’s release may be required to return to work. Failure to maintain a satisfactory attendance record may result in disciplinary action, up to and including termination of employment. Working all scheduled hours is essential to the ability to meet the needs of staff and patients. Employees who are excessively tardy and/or absent may be subject to disciplinary action, up to and including termination of employment. Workday/Meal Break Expectations Each department and work location will determine work schedules for each employee to include breaks and mealtimes, as appropriate. Any break or mealtime lasting 20 minutes or less will be paid. Any break or mealtimes more than 20 minutes will not be paid, provided the employee was free to leave the work premises, performed no work, and

The Company’s attendance policy is considered a “no fault” attendance program. This means that a doctor’s statement/excuse will not make an absence excused. All absences impact COPC, as it relates to the Company’s ability to meet the needs of its patients. The Company reserves the right to request a doctor’s release to allow an employee to return to work. Please note, some work locations and departments have more specific attendance policies, which are in addition to this policy and must be followed accordingly. It is a condition of employment for every employee to attend work as scheduled and report to work on time. It is never acceptable for an employee to willingly “No Call/No Show” to their job. Employees who do so may be subject to disciplinary action, up to and including termination of employment.

had no work responsibilities during the break or meal period. Unpaid breaks and mealtimes more than 20 minutes will not count towards hours worked for the purposes of overtime calculation.

5.3

Payroll Work Week and Pay Schedule

The standard work week commences Sunday at 12:00 a.m. and ends at 11:59 p.m. on the following Saturday. All employees of the Company are paid bi-weekly (26 pays per year) for time worked during the previous 2 week’s pay period. Compensation will be electronically deposited into the employee’s bank account or mailed to their office or home if they do not have a bank account. When a payday falls on a holiday, compensation will be electronically deposited into the employee’s bank account the preceding business day.

If an employee believes there may be an error in their paycheck, they should promptly notify Payroll@COPCP.com so that it may be addressed in a timely manner. Should it be determined that an error occurred, every attempt will be made to ensure the correction is reflected in the next regularly scheduled paycheck.
