Your Assurant Dental plans focus on prevention and early diagnosis providing two cleanings per year.
Buchanan Hauling is pleased to announce that SunLife Dental will continue to be our dental benefit provider for 2018-2019 plan year. The SunLife Dental plan provides 100% coverage of preventive services. You pay a percentage of costs for other types of dental care you receive. The percentage you pay depends on the type of care: Preventive, Basic, Major, or Orthodontia and the plan you select. If you see a network dentist, you can simply provide your SSN and they will take care of the paperwork for your visit. If your out-of-network dentist requires you to pay up front for services, you will need to submit a claim form for reimbursement for covered amounts.
