o www.myuhc.com Choose Benefits, then select network “Choice” for POS plans HOW THE PLAN PAYS • For all of the health insurance plans preventive care is covered 100%. • The PPO plans offer prescription drug coverage with no deductible. With the HSA you will need to pay out of pocket (with pre-tax HSA dollars) for prescription drugs until you meet your deductible. Once your out-of-pocket expenses reach the annual maximum, all plans will pay 100%. Go to your www.myuhc.com or www.welcometouhc.com PRESCRIPTION DRUG BENEFITS Whether you choose the PPO, POS or HDHP/HSA plans, prescription drug coverage is included. Your cost for a prescription depends on the type of drug you receive and where you obtain it. Under the HDHP/HSA plans, you pay the full cost, either out of pocket or with pre-tax HSA dollars, until you reach the deductible. Prescriptions can be filled at any UHC network pharmacy which includes many major chains and independent pharmacies. PRESCRIPTION BENEFIT PROGRAM – HOW TO SAVE MONEY: Prescription drugs can be quite expensive, especially if you have one or more maintenance prescriptions.
