u Could this have been avoided? u This accident happened just after lunch. u Repetitive jobs, mind wandering, concentration, cut corners? u Complacency sets in? u Task Risk assessment? Toolbox talk? u Safety training? u PPE? u The lesson here is that life threatening risks exist in our daily lives as we walk round vendors or fab sites from dropped objects and, like in this case, risk of crushing. Many times it may not even be our fault. We may just be in the wrong place at the wrong time. u This example shows that even with a strong safety culture, when all the procedures are in place accidents can still occur. u This is a reminder to be vigilant at all times when in high risk situations and remember you are your own Safety Officer.

• Begin site Inspections of piping and equipment with P&IDs and • datasheets. • Electrical schematics. • This activity continues until the plant is mechanically complete. • Maintain Punch List of construction deficiencies. • Confirm vendor support and schedule for startup activities. • Deliver or coordinate Operator Training. • Update schedule and procedures as necessary.

• Continue Site Inspections with Operations and vendors. • Witness equipment inspections and closure as appropriate. • Update Punch List. • Coordinate Site Acceptance Testing of controls equipment. • Coordinate and perform as necessary pre-commissioning activities (line flushing, loop checks, leak checking, etc.) and witness as appropriate. • Coordinate loading of catalyst / mol sieve / etc. • Begin Turnover of Systems from Construction to Operations.

• Complete turnover of all systems from Construction to Ops. • Maintain Final Punch List (typically B and C items). • Coordinate commissioning of all utilities. • Assist with air-freeing of all gas systems. • Coordinate dryout and cool-down of cryo units. • Coordinate dryout of refractory systems. • Provide technical support for feed introduction and initial operations and coordinate with vendors as necessary. • Coordinate Site Acceptance • Testing of mechanical equipment.
