What Older Adults Should Know After A Doctor’s Visit
(StatePoint) Y ou know how important regular check-ups are and that consulting your healthcare provider when you have a concern is smart. But what happens after the visit is just as essential to maintaining good health. Unfortunately, some people don’t follow their provider’s advice. Maybe they don’t take their prescriptions as written or forego recommended preventive screenings and immunizations. This can be costly, both for individuals and the health care system. Case in point: More than 125,000 people die each year because of prescription medication non-adherence, according to the National Council for Patient Information and Education. Following a doctor’s recommended care plan can be challenging, according to Dr. J.B. Sobel, chief medical officer with Cigna Healthcare’s Medicare business. There are multiple reasons why older patients can have difficulty following their provider’s instructions – including managing multiple medications at different times of day – or trouble accessing or affording care. Thankfully, there are some processes and resources that can help you faithfully follow your healthcare provider’s advice. Get organized. More than half of adults 65 and older take four or more prescription drugs, according to the Kaiser Family Foundation. With more medications, it can be difficult to know what to take, when to take it, and in what dosage. A low-cost pill organizer, available at drug
medication doesn’t seem to be working or is causing undesirable side effects, talk to your doctor or pharmacist about adjusting your dosage or switching to an alternative. Likewise, discuss any concerns about screenings or tests with your provider. Having a good rapport with your doctor helps. Take notes at your appointment or bring a loved one with you to help with questions. And don’t be afraid to ask if there’s anything you don’t understand. Connect your providers. Make sure your various doctors share information. For example, if you have an unexpected hospital visit, notify your primary care provider so they can review any new medications or diagnoses from the hospital and help you fit them into your current care plan. This can also help protect you from unnecessary or duplicate procedures. Find alternative access to care. If you have difficulty getting around or live far from where care is rendered, there are programs to help. Your provider may offer virtual visits. You may be able to have prescriptions safely delivered to your home in a 90-day supply, perhaps at a lower cost, and with reminders for refills. If you have a Medicare Advantage
Advantage plans at www.cigna.com/ medicare. Seek help with costs. “Extra Help” is a federal program providing prescription drug cost support to those who qualify. Many pharmaceutical manufacturers help customers afford medications. You could also ask your healthcare provider about generic medications, which typically cost less but are equally effective. Medicare and Medicare Advantage plans pay for many preventive procedures with no extra cost to you. “If keeping up with your health care feels overwhelming, please don’t get discouraged,” said Dr. Sobel, who oversees a team of clinicians that contact Cigna Healthcare’s Medicare Advantage customers whose prescriptions have lapsed or not been refilled. “Seek help from a doctor, a pharmacy or a loved one. They want to help you. Pain In Your Foot or Ankle? It Could Be Athritis
(StatePoint) A rthritis in the feet and ankles can produce swelling and pain, deformity, loss of joint function and loss of mobility. Whereas previous generations had to accept this as a normal part of aging, an explosion of new therapies and surgical treatments is offering patients today both hope and relief. For best results however, foot and ankle surgeons urge early intervention. “When it comes to arthritis, it’s important not to tough out symptoms or bear the pain,” says Danielle Butto, DPM, FACFAS, a foot and ankle surgeon and Fellow member of the American College of Foot and Ankle Surgeons (ACFAS). “Earlier treatment is not just about alleviating symptoms sooner. In many cases, we can even slow the progression of the symptoms, and use less invasive procedures to treat the condition than we would otherwise.” Understanding the early warning signs of arthritis, the progression of different forms of the disease and the new treatments available are important for getting the proper treatment and managing your symptoms. Osteoarthritis Osteoarthritis is a degenerative condition characterized by the breakdown and eventual loss of cartilage in the joints. One common area where osteoarthritis occurs is the big toe. The big toe makes it possible for you to walk and run upright, absorbing forces equal to nearly twice your body weight when walking. With all it endures, it’s no surprise that overuse can erode cartilage, causing serious pain and even physical deformities. Nevertheless, many people confuse big toe arthritis, also known as hallux rigidus, with bunions. Early signs of hallux rigidus include pain and stiffness during use, or during cold, damp weather, difficulty with activities like running and squatting, and swelling and inflammation around the joint. Additional symptoms may develop over time, including pain during rest, bone spurs, limping, and dull pain in the hip, knee or lower back due to changes in gait. If you notice any of these symptoms, see

a foot and ankle surgeon. Conservative treatments like shoe modifications, orthotic devices and physical therapy, may prevent or postpone the need for surgery. If you have mid- to end-stage arthritis in your foot or ankle, you may require next steps, such as ground-breaking cartilage regeneration treatment, implant surgery or the surgical removal of damaged cartilage and spurs, all of which have quick recovery periods. “People with this disorder tend to suffer much longer than they need to,” says Dr. Butto. “They’re often pleasantly surprised when they find out their problem can be fixed.” Rheumatoid Arthritis Rheumatoid arthritis (RA) is a disease in which certain cells of the immune system malfunction and attack healthy joints. Foot problems caused by RA commonly occur in the ball of the foot near the toes, although RA can also affect other areas of the foot and ankle. The most common signs and symptoms are pain, swelling, joint stiffness and difficulty walking, as
well as a range of deformities, including rheumatoid nodules, dislocated toe joints, hammertoes, bunions, heel pain, Achilles tendon pain and flatfoot ankle pain. As part of a team that includes a primary doctor or rheumatologist, your foot and ankle surgeon will develop a treatment plan aimed at relieving associated pain, which may include orthotic devices, accommodative shoes, aspiration of fluid build-up and steroid injections. When RA produces foot pain and deformities not relieved through other treatments, surgery may be required. “Listening to your body and seeing a foot and ankle surgeon are so important. With today’s scientific advances, treatment can reduce pain and restore you to your previous mobility, strength and functionality,” says Dr. Butto. 6 Ways To Build Lasting Healthy Habits
(FamilyFeatures) S tarting on a path toward healthy habits is often easier than maintaining them long term. This year, you can avoid a major pitfall of healthy resolutions and build healthy habits that stick by working small, positive steps into your daily life. In fact, healthy habits are the first suggested treatment strategy for people whose blood pressure and cholesterol levels are creeping higher than normal, according to an American Heart Association scientific statement. “The current guidelines for managing high blood pressure and cholesterol recognize that otherwise healthy individuals with mildly or moderately elevated levels of these cardiovascular risk factors should actively attempt to reduce these risks, and increasing physical activity is a great place to start,” said Bethany Barone Gibbs, Ph.D., chair of the statement writing group and chair of the department of epidemiology and biostatistics at West Virginia University School of Public Health. These six ideas from the American Heart Association’s Healthy for Good Habit Coach can help. Bust Common Habit-Building Myths You may be surprised to learn the truth about creating and sticking to healthy habits. One myth is getting healthy means doing things you don’t like. Research shows positive emotions make habits stick, so set your intentions on something you enjoy. Another misconception is big results require big changes, which may lead to overly ambitious habits. However, the simpler the routine is, the more likely it is to become habit. Work with Your “Brain Loops” Your brain creates “loops” for habits made up of three things: a cue, a routine and a reward. Each time the loop is repeated, it becomes more routine and may become automatic. Knowing this, you can design cues for developing new, healthy habits, such as setting walking shoes by the bed to start a walking habit. The routine is putting on the shoes and walking around the block, and the reward is the pleasant sensations and
brighter mood from a morning stroll. Create Cues That Work for You Most successful health habits begin with a cue. The cue can be external in your environment or internal in terms of your mindset. The more consistent the cue, the more likely it is to trigger the habit. Hacking your brain’s reminder system can help you remember your cue. Some examples of visual cues are placing a sticky note where you’ll see it often, keeping a water bottle on your desk or refrigerating fresh veggies at eye level. Build a Routine That Supports Your Goals Positive and consistent habits are important to achieve your personal goals. Small habits done consistently can add up to big results. To create a new healthy habit, think through the steps that could lead to your desired outcome. Ask yourself whether you want to do it, if it’s easy and if it’s high impact. It’s important to choose habits that make a difference and move you closer to your goals. For example, if one of your goals is improving your heart health, a meaningful habit might be to move more. Increasing physical activity can help lower blood
pressure and cholesterol along with many other health benefits, Gibbs said. “Every little bit of activity is better than none,” she said. “Even small initial increases of 5-10 minutes a day can yield health benefits.” Use Rewards to Make Habits Stick Start by choosing a habit you enjoy that’s rewarding by itself. If you’re more of a dancer than runner, increase your physical activity with an upbeat dance class. You might also look for a more enjoyable version of a new habit, such as getting more fruits and veggies by sipping on a delicious smoothie. Understand Resets are Part of the Process New habits are experiments. If they don’t stick, you haven’t failed. Instead, you’ve learned what doesn’t work, which is useful. Get curious and ask yourself which part of the habit didn’t work for you. Maybe the cue was ineffective. Maybe the steps of the routine were too ambitious and you need to split them into smaller, easier steps. If you realize you don’t enjoy the habit, stop doing it and try something else.
