What are the practical steps to achieve connected mental health care?

T hrough Nous’ work in with stakeholders across the globe, it is clear that the current approach to helping people to find, access and use mental health services is not working. Services have evolved in silos, shaped by different policies, funding models, and organisational priorities. As a result, people Australia, Canada and the UK, and in our conversations seeking help often must navigate a maze of disconnected pathways, facing repeated assessments and dead ends. In addition, people often require multiple types of support across adjacent systems that currently don’t speak to each other. The result? Poor experiences and delayed support for those who need it most. But there is hope, through a widely agreed vision: Care that is connected, holistic, and person centred – often described as ‘connected care’. Coordinated investment across regions globally is supporting the delivery of connected care, with progress occurring at various stages. For example, in Australia, the National Healthcare Interoperability Plan outlines a clear pathway toward connected care, supported by My Health Record, Provider Connect, the National Health Services Directory, Healthdirect and Health Connect.

In the EU, the European Health Data Space (EHDS) supports secure and consistent health data exchange across all member states. In Singapore, the National Electronic Health Record (NEHR) aggregates patient information from various healthcare providers, creating a comprehensive view of an individual’s medical history and streamlining coordination across care teams and clinical decision-making. However, to achieve connected care for mental health we need to do two things. First, ensure these enabling capabilities appropriately support mental health use cases. Second, move from organisations trying to address this issue on their own, to working collectively towards our common goal. How do we achieve connected care?
