Funding and commissioning Are our funding and incentives structured to reward collaboration and system-wide outcomes, not just activity? Enabling infrastructure investment Are we investing in the foundational enablers like digital infrastructure and workforce capability—that make connected care possible? Delivery Are we using government run services to model and accelerate best practice in connected care? Governments must ask themselves how they are creating the right conditions for connected care to become a reality using all available levers, and working in new ways with service providers, other government organisations, and researchers. Connected care is a long-term vision and will take time and sustained commitment to get right. It is important that as we work toward this vision, that we work collectively, supporting all organisations to think differently and more collaboratively about how we support people to find and access care. The enablers are in place, and all parts of the system are on board. What needs to happen now is action at all levels.
