1 Currently, the demand for CCW enrollment exceeds capacity. Unless there is an emergency, there may be a multi-year wait before accessing services through the CCW. 2 Individuals who are eligible for the CCW may access the Supports Program while waiting for CCW services (p. 31). 3 Some individuals may still be working with case managers. Going forward, virtually all individuals will eventually transition to Support Coordination Agencies. 4 If a nursing home level of care is met, Managed Long-term Services and Supports (MLTSS) waiver or state plan services may be an option.
