They can help support consumer /family into the transition to the Self-Determination Program or when already in SDP SDP Coaching is available to provide additional education and support on the Self Determination Program funded by NLACRC You can reach out to your CSC for a referral or go directly to SDP Coach: Claudia Cares AACColades Autism Society Help Grow Integrated Community Collaborative (ICC) - available to coach up to 1/31/2026
