Fungus Among Us: Diagnosing and Managing Valley Fever in Practice by Dr. Jared Jaffey, DACVIM (SAIM) • Valley Fever without the fever? Despite the name, fever only occurs in about half of cases. • Even if Valley Fever isn’t endemic in your backyard, remember — it only takes a couple of spores to hitch a ride. A quick drive through an endemic zone can be enough for exposure, so keep this fungus on your radar no matter where your patient calls home. • Valley Fever tends to peak in late summer through winter. • The silver lining? Valley Fever isn’t contagious between animals or people so no need for isolation protocols when hospitalizing an infected patient. • Listen to the history: desert walks, digging, or even an indoor cat near open windows. Spores travel on dust, dirt, and people — so no pet is truly at a “low risk.” • A vaccine may soon be available to help lessen clinical signs and disease severity if infection should occur. • Antigen testing can miss up to 70% of cases; AGID antibody testing remains the go-to for diagnosis and monitoring. • If tracheobronchial lymph nodes appear enlarged on radiographs, keep Valley Fever near the top of your list. Just remember - this fungus is a master of disguise with no one classic look on radiographs or routine bloodwork panels.
