Complete Medical Report Form found in ERNIE > Tools > Medical/Health Portal > My Forms. Provide proof of measles, mumps and rubella immunizations, hepatitis B injections and a meningococcal vaccine. This must be on file in the Wellness Center prior to orientation to avoid a registration hold, housing check-in hold, inability to pay bills and other holds. Health insurance is mandatory. All students are automatically enrolled in the university policy, and the cost will be added to your student account. If you have comparable health insurance and want to opt out, you must complete a waiver through ERNIE. Watch your Student Homepage Task tiles for “Health Insurance Waiver/Election” to appear as an action item.
