The City of Midland’s 2024 UMR Vision plan ended on December 31, 2024. Your deadline to submit a vision reimbursement claim(s) is a year from the date of service. UMR Vision (plan year 2024) CLICK HERE: Member Vision Claim Submission Form All submissions must include an itemized statement of services, the provider’s tax identification number, and costs (including the CPT codes). As of January 1, 2025, the vision plan is administered by Ameritas.
