Friends and Family Test Cards
Gathering Feedback At WUTH, we ask patients to fill out a Friends and Family Test (FFT) card. This short survey helps us improve care at the hospital. On the card, you answer one main question: "Overall, how was your experience of our service?" You can fill out several FFT cards during your stay. Please mention a staff member by name if you can. Patients, visitors, birthing partners, and carers can also fill out the FFT cards.
Why We Ask Demographic Questions The card also asks some questions about you, like your age and ethnicity. This helps us see how different groups are rating our care, so we can make it better for everyone. These questions are not to identify you as a person. Please take a few minutes to fill out a card. You can find them on your ward, or ask a staff member. You can fill out more than one card if you want to share feedback about different services or different days you are here. Your anonymous feedback is very valuable. It helps us make care the best it can be for you and future patients.
