Identity Verification We need to be confident that we are doing business with a real person and stop applications with falsified personal information. Every application is checked against a national database of public information. If we cannot find a match, the application cannot be accepted. Lookback Period In order to apply for TruStage underwritten life insurance products, the applicant must authorize us to check databased for information such as prescription and laboratory history. These database checks typically retrieve any relevant records from the past seven years. These records are automatically analyzed and assigned a mortality score based on factors including but not limited to recency, frequency and severity of the information. Prior Declined Applications If an individual is declined for coverage due to health/underwriting risk, they are not eligible to re-apply for that product for 30 days. Every application is checked against previously declined applications from the past 30 days. If there is a match, the application cannot be accepted. Maximum Per-Life Coverage Limits Each TruStage Life product has a maximum amount of in-force coverage allowed for any individual person (see below for specific limits). There is no limit to the number of separate policies for any one person, provided that the total coverage does not exceed the maximum limit. Every application is checked against active policies as well as pending applications. If the new application would put the total coverage over the limit, the application cannot be accepted. The applicant may proceed with an application for a lower coverage amount if it does not exceed the limit. In all cases, the coverage sold must fit the needs and budget of the applicant.

TruStage™ Term Life Insurance is issued by MEMBERS Life Insurance Company. The insurance offered is not a deposit, and is not federally insured, sold or guaranteed by any financial institution. This is a term policy to age 80 that has premiums that increase when entering each five-year band: 25, 30, 35, 40, 45, 50, 55, 60, 65, 70, and 75. Approval is based on your answers to the questions on the application and additional information we obtain from other sources. Rates may be higher due to tobacco use. Base Form: ICC25-PBT1, 2025-PBT1. TruStage™ Guaranteed Acceptance Whole Life Insurance is issued by CMFG Life Insurance Company. Death benefit is limited the first 2 years of the policy and is limited to a return of 100% of premiums, plus 10%. Base Form Numbers: ICC16-GAWL, 2016-GAWL. TruStage™ Advantage Whole Life Insurance is issued by MEMBERS Life Insurance Company. Base Form Numbers: ICC20-WLMLIC, 2020-WLMLIC.
