HIT5-8B Overview of the requirement:
Client/patient medications, both prescription and nonprescription, are reviewed on an ongoing basis by a registered nurse (RN), pharmacist, or licensed skilled professional. Compliance is assessed through observation and review of policies, procedures, and client/patient records. Most deficiencies stemmed from lack of documentation in the medication profile, especially concerning frequency, route, and dose.

Examples of surveyor findings:
n Medication profile included saline and heparin flushes “as directed,” but did not specify a frequency for these medications to be administered. n The medication profile does not include the route for the medication. n Medication frequency was documented “As needed” without a reason. n Medication profile did not include a diluent, total volume, method of infusion, rate of infusion and specific directions. n Medication profile included a frequency of 5 days for 3 weeks, but there were no dosages of these medications. n Medication profile does not include all medications patient is currently taking. It only includes medications the home infusion nurse is administering.
Tips for compliance:
n A complete medication profile consists of: All current medications. Date prescribed or taken. Medication name. Dose. Route. Frequency. Date discontinued. Allergies.
n Adopt and enforce a written policy for maintaining a current medication profile. Educate staff to review medications in the home to ensure accuracy. n Audit client/patient records for completeness of the medication profile.
