Evidence of excellence in health care improvement science include: 1. Clinical systems excellence that is sustained and replicated and which results in breakthrough results locally andnationally. 2. Implementation of improvement outcomes that are nationally and internationally distinctive, especially those that involve multi-site collaborative/networks. 3. Publication and dissemination of quality improvement, safety or health care outcome projects, service on expert panels, consultant on QI projects outside of this academic health center. 4. Membership on editorial boards, editorship of QI journals and text books. 5. Leads regional or national improvement work across institutions and health systems. Evidence of excellence in clinical scholarship, service and/or administration include: 1. A commitment to life-long learning through participation and leadership of postgraduate activity (e.g. national conferences, workshops and continuing education). 2. Participation on editorial boards as associate or deciding editor and as a reviewer for high caliber journals relevant to the faculty candidate’s research and clinical interest. 3. Mentorship of junior colleagues as demonstrated by their evaluations, successes and advancement. 4. Evaluations from the candidate's peers, colleagues, students, patients, division chief, and department director outlining the clinical and service and/or administration capabilities of the individual. This may include physicians who refer patients to the faculty member, as well as colleagues who work closely with him or her, have a particularly valuable perspective. Prominence at this rank may include regional, national or international referral and outcomes based on evaluation from the referring physicians. 5. Local, regional and national and international recognition of research and clinical expertise, including invitations for speaking at research and clinical seminars, courses, and symposia of national and international prominence. 6. Leadership in local, national and international professional organizations. 7. High profile at national meetings, such as serving as a conference organizer or session chair.
